Very little is know about Spenny's grandmother. In the mini-competitions episode, there is a mini-competition where they have to see who can produce the most sperm, Kenny produces his, but leaves an image of Spenny's grandmother in the room to put Spenny off. When Spenny goes in, he finds the picture and it does put him off. So Spenny storms out with the picture, shows it to Kenny and demands he apologies to all of his family.
